Your health care is very important to us. Prior to enrollment, a HEALTH FORM is sent to all incoming students. This is a tool to assist us plan your health care needs. It is important that this form be returned to the Health Center in its entirety prior to August 1, 2007. 

The California State Department of Health Services and the University requires students to have a completed immunization record against all vaccine-preventable diseases as a condition of enrollment. If documentation of immunization is not available, re-vaccination is required.  Contact the Student Health Center at 626-395-6393 if you need further assistance.
