Dental Information

All registered students are eligible to enroll in the Delta Dental HMO Plan during the enrollment window every September.  (The Student Health Insurance Plan does not cover dental care.)  The Delta Dental HMO plan is a managed-care dental program.  There are no deductibles and many routine dental expenses are fully paid by the plan.  Other treatments require set co-payments.  When you enroll, you select a Delta Dental HMO network dentist.  For more information about the plan benefits and providers please see below:

Please submit your enrollment form or dental plan waiver to the Benefits Department at MC 161-84 by October 3, 2008.  If you have any questions please contact the Benefits Department at extension 6443.

If you have any questions regarding the dental plan coverage or dental providers please contact DeltaCare at 800-422-4234.  The undergraduate student plan number is 05597-001, or graduate student plan number is 05597-0002.
