If you were born after 1956 you must provide the following information even if it differs from the practices in your own state or country of origin.

• For measles:  2 dates of vaccine administration
• For mumps and rubella : one date of vaccine administration
• If no documentation is available, a titer result (serology) and physician signature confirming immunity to the disease is required
• Physician documentation of illness is not acceptable
